The Nepali-specific difficulty
Nepali has a smaller pool of published practice material than Hindi or Punjabi, so candidates over-practise the handful of dialogues that circulate and get caught out by unfamiliar domains on the day. We deliberately drill domains that are under-represented in the free material.
Honorific levels are the second issue. Nepali marks social relationship through verb forms, and candidates who default to one register throughout a dialogue lose marks when the speakers clearly stand in different relationships to each other.
Third is technical vocabulary in health and legal settings, where the Nepali term exists but sits outside daily use, and candidates substitute an English word rather than the expected equivalent.
